Insurance Options

We accept most major insurance plans.  We also match in-network benefits for those plans with whom we are currently out-of-network. Our staff will be happy to assist you in determining benefits at our office.

Worker’s Compensation Insurance: We do accept worker’s compensation insurance for those who have been injured on the job. Please fill out the Workman’s Compensation Form. If your claim is denied, the charges are your responsibility, and we will work with you accordingly.

Automobile Accident: If you have been involved in a car accident please fill out the Motor Vehicle Accident Information form. If your claim is being handled by an attorney, please bring that information with you.

Uninsured Patients: We are willing to make arrangements with you if you don’t have medical insurance. Please consult with us to work out a payment plan and discounted rate.
